Voluntary benefits — deadline to enroll is Nov. 30

While completing Open Enrollment for your 2009 benefits*, take some time to review the available voluntary benefit options. Once you complete, submit, and print your Open Enrollment selections, you will be given the opportunity to review, enroll, or make changes in the voluntary benefit programs by clicking the Continue Enrollment button.

If you have already completed your Open Enrollment elections, you can still access and enroll in the voluntary benefit programs until Sunday, Nov. 30, by logging in to My HUB, clicking on the Human Resources tab, and then clicking on My Voluntary Benefits under My Benefits.

Voluntary benefit options include:

• AFLAC
• Personal Short-Term Disability
• Personal Accident Expense Plan
• Specified Health Event Protection
• Cancer Policy
• ARAG Legal Insurance Plan
• UNUM Interest-Sensitive Whole Life Insurance

These voluntary programs are offered to UPMC employees at discounted group rates negotiated with the various carriers.

Employees currently enrolled in an AFLAC program who want to cancel the policy for 2009 must select the document section at the top of the screen inside My Voluntary benefits and download the AFLAC Policy Cancellation Form. This form needs to be completed and faxed to AFLAC at the fax number provided on the form by Nov. 30.

Employees currently enrolled in the Unum Interest-Sensitive Whole Life Insurance policy who want to cancel their policy for 2009 must download the Unum Customer Service Request form, complete it, and fax to Unum.

*Benefits and eligibility described may not be applicable to all employees. Some business units have unique benefit programs, certain job classifications may affect benefit eligibility, and contract terms determine benefits for physicians and members of collective bargaining units.
