Flu shot reimbursement

UPMC offers flu vaccinations at no cost to all staff and physicians. If you paid to receive a flu shot from a non-UPMC facility or event, be sure to submit a completed Flu Shot Reimbursement form, which can be found on the flu site on Infonet.
